Forms Needed to Register

Registration is on-line at our registration tab: http://www.gwsoccer.com/registration/index_E.html. You will need to sign up for travel tryouts and then once accepted by the club you can go back in and register for travel.

Other than that, we will need the following:

IWSL Registration Form

IYSA Medical Waiver form

a copy of the birth certificate or passport

digital pictures of the athlete’s face.
